From the 1 December we can no longer accept disclosure applications that are sent to us on the old form.  <\/p>\n<p>Please make sure that you start using the new forms and destroy all stocks of the old form. <\/p>\n<p>The new form can easily be identified as the form reference number at the top right-hand side begins with F01.
