{"id":76827,"date":"2016-12-04T00:15:22","date_gmt":"2016-12-04T00:15:22","guid":{"rendered":"http:\/\/mostafa.openonline.co.uk\/?guid=5fd59b680d829c5ae409b8fbc2c9ec5a"},"modified":"2016-12-04T00:15:22","modified_gmt":"2016-12-04T00:15:22","slug":"press-release-nhs-england-and-phe-extend-hiv-prevention-programme","status":"publish","type":"post","link":"https:\/\/mostafa.openonline.co.uk\/?p=76827","title":{"rendered":"Press release: NHS England and PHE extend HIV prevention programme"},"content":{"rendered":"<div class=\"govspeak\">\n<p>NHS England will fund a major extension to the national <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr> prevention programme led by Public Health England (<abbr title=\"Public Health England\">PHE<\/abbr>) with the aim of supporting those most at risk and reducing the incidence of <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr> infection.<\/p>\n<p>It has also decided to routinely commission 10 new specialised treatments as part of the annual prioritisation process for specialised treatments.<\/p>\n<p>The new <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr> initiative is joint between NHS England and <abbr title=\"Public Health England\">PHE<\/abbr>, and follows the recent Court of Appeal ruling that NHS England, alongside local authorities, has the power, although not the obligation, to fund the provision of anti-retroviral drugs for the prevention of <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr>, known as pre-exposure prophylaxis (<abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr>).<\/p>\n<p>The first phase of implementation will be the launch of a large scale clinical trial early in the 2017 to 2018 financial year. Although the evidence around the clinical effectiveness of <abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr> is strong, advice from <abbr title=\"Public Health England\">PHE<\/abbr> has highlighted significant outstanding implementation questions that should be answered prior to using <abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr> in a sustained way on a substantial scale in England. These questions will be answered by the clinical trial, paving the way for full roll-out.<\/p>\n<p>It is anticipated that the clinical trial phase will include at least 10,000 participants over the next 3 years. NHS England will fully fund the cost of the clinical trial phase and will work in partnership with local authorities, the Local Government Association (<abbr title=\"Local Government Association\">LGA<\/abbr>) and <abbr title=\"Public Health England\">PHE<\/abbr> to implement the findings as part of a wider national roll-out.<\/p>\n<p>Detailed planning will now take place to ensure the launch and the clinical trial phase can begin as swiftly as possible. Up to \u00a310 million will be made available over the next 3 years to fund all aspects of the trial. Next steps will include asking both the manufacturer of the branded <abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr> drug Truvada, as well as generic manufacturers to make proposals to participate in the trial.<\/p>\n<p>Dr Jonathan Fielden, Director of Specialised Commissioning and Deputy National Medical Director, NHS England said:<\/p>\n<blockquote>\n<p>We\u2019re pleased to be able to announce funding not only for 10 new specialised treatments but also a new ground breaking national programme for <abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr> that will benefit at least 10,000 people.<\/p>\n<p>This has, in part, been made possible by the willingness of many pharmaceutical and device companies to come forward with lower and more responsible prices. Continuing this constructive joint working will enable us to fund more new drugs and treatments in the future.<\/p>\n<p class=\"last-child\">We have, however, had to make some tough decisions over what we are not able to fund at this point in time within the resources we have available. We will ensure those treatments have the opportunity to be considered as part of the next annual prioritisation round in spring 2017.<\/p>\n<\/blockquote>\n<p>Professor Kevin Fenton, Director of Health and Wellbeing at <abbr title=\"Public Health England\">PHE<\/abbr> said:<\/p>\n<blockquote>\n<p>Currently 13,500 people are living in the UK with undiagnosed <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr> and we are still seeing around 5,000 new infections each year. Given we are in the fourth decade of this epidemic there are too many new infections occurring, and we need to use all tools available to save lives and money. We\u2019re delighted to be working in partnership with NHS England on this major new addition to the national <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr> prevention programme. This comes after much planning and preparation to ensure we can successfully coordinate this extremely important and large scale clinical trial.<\/p>\n<p class=\"last-child\">We encourage all those who may be at risk of <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr> to ensure they get tested and we are again working with local authorities to fund the <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr> home-sampling test kit as well as issuing joint guidance for the first time with <abbr title=\"National Institute for Health and Care Excellence\">NICE<\/abbr>, which supports increased uptake of <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr> testing.<\/p>\n<\/blockquote>\n<p>Councillor Izzi Seccombe, Chairman of the <abbr title=\"Local Government Association\">LGA<\/abbr> Community Wellbeing Board, said:<\/p>\n<blockquote>\n<p>We are pleased that NHS England has acted quickly and chosen to fund the commissioning of this trial and roll-out of <abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr>. We now want to stand united with the NHS to defeat the spread of <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr>.<\/p>\n<p><abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr> is a ground-breaking method of treatment that has the potential to save lives. Councils want to work with the NHS to help roll-out the trial.<\/p>\n<p class=\"last-child\">Local authorities have invested millions in providing sexual health services since taking over responsibility for public health 3 years ago, and we firmly believe that <abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr> could significantly reduce levels of <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr> in the community.<\/p>\n<\/blockquote>\n<p>Dr Ian Williams, Senior Lecturer and Hon Consultant Physician, Central and North West London NHS Foundation Trust and Chair of NHS England\u2019s Clinical Reference group for <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr> said:<\/p>\n<blockquote>\n<p>This announcement demonstrates NHS England\u2019s commitment to fund <abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr> and provides the chance to best prepare England for optimal roll-out following this large scale clinical trial. For now, the trial will provide access to <abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr> for thousands of people most at risk of acquiring <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr>.<\/p>\n<p class=\"last-child\">I\u2019m delighted that the work of many people from the <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr> Clinical Reference Group, including clinicians and patient advocates, is providing the foundations for this trial. Now we look ahead to working with partners in <abbr title=\"Public Health England\">PHE<\/abbr>, the voluntary sector and local government to get the trial underway across England, helping us to better understand how to integrate <abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr> as part of comprehensive <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr> prevention service aimed at preventing transmission of <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr> and other <abbr title=\"Sexually transmitted infections\">STIs<\/abbr>.<\/p>\n<\/blockquote>\n<p>The decision to routinely commission 10 new specialised treatments is based on advice from NHS England\u2019s clinical priorities advisory group which assessed the relative priority of investing in a new range of specialised treatments and interventions. This took into account revised prices submitted by some manufacturers since provisional investment decisions were published in July.<\/p>\n<p>The treatments that will now be routinely commissioned are:<\/p>\n<ul>\n<li>Pegvisomant for acromegaly as a third-line treatment for adults<\/li>\n<li>Auditory brainstem implants for congenital abnormalities of the auditory nerves or cochleae<\/li>\n<li>Haematopoietic stem cell transplant &#8211; Lymphoplasmacytic lymphoma \/Waldenstrom\u2019s Macroglobulinaemia (adults)<\/li>\n<li>Everolimus for subependymal giant cell astrocytoma (SEGA) associated with tuberous sclerosis complex<\/li>\n<li>Rituximab for immunoglobulin-G4 related disease (IgG4-RD)<\/li>\n<li>Microprocessor controlled prosthetic knees<\/li>\n<li>Tolvaptan for hyponatraemia secondary to the Syndrome of Inappropriate Antidiuretic Hormone (<abbr title=\"Syndrome of Inappropriate Antidiuretic Hormone\">SIADH<\/abbr>) for patients who require cancer chemotherapy<\/li>\n<li>Ivacaftor for children (2 to 5 years) with cystic fibrosis (named mutations)<\/li>\n<li>Sodium oxybate for symptom control for narcolepsy with cataplexy (children)<\/li>\n<li>Pasireotide for Cushing\u2019s Disease<\/li>\n<\/ul>\n<p>Both the decision to fund a new national <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr> prevention programme and to routinely commission the 10 new treatments follow consideration by NHS England\u2019s specialised services commissioning committee.<\/p>\n<h2 id=\"background\">Background<\/h2>\n<p>The <abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr> clinical trial will answer questions raised by <abbr title=\"Public Health England\">PHE<\/abbr> about 6 important outstanding questions:<\/p>\n<ol>\n<li>What proportion of genitourinary medicine (GUM) clinic attendees will be assessed as eligible for <abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr>?<\/li>\n<li>How to identify, engage and maintain other eligible <abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr> users?<\/li>\n<li>What proportions of the eligible will accept <abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr> and will choose daily or intermittent dosing?<\/li>\n<li>For how long will those beginning at high risk stay on <abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr>?<\/li>\n<li>What impact will <abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr> have on <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr> incidence?<\/li>\n<li>What impact will <abbr title=\"pre-exposure prophylaxis\">PrEP<\/abbr> have on <abbr title=\"Sexually transmitted infection\">STI<\/abbr> incidence?<\/li>\n<\/ol>\n<p><abbr title=\"Public Health England\">PHE<\/abbr> and <abbr title=\"National Institute for Health and Care Excellence\">NICE<\/abbr> have also announced the roll-out of new <a rel=\"external\" href=\"https:\/\/www.nice.org.uk\/guidance\/NG60\">guidance to increase the uptake of <abbr title=\"Human immunodeficiency virus\">HIV<\/abbr> testing<\/a>.<\/p>\n<p><abbr title=\"Public Health England\">PHE<\/abbr> has been working closely with <a rel=\"external\" href=\"http:\/\/www.ssat.org.uk\/\">St Stephen\u2019s AIDS Trust<\/a> in developing the trial proposal.<\/p>\n<p>The 3 proposals with the lowest cost and benefit priority are not currently affordable and will not be routinely commissioned at this time.
