This caused the outer pane of a window to be broken and caused some underframe damage. No one was hurt. The trolley was one of four that had been stored in the tunnel since the night of 4\/5 December.<\/p>\n<p>We have undertaken a <a href=\"https:\/\/www.gov.uk\/guidance\/raibs-response-to-accident-and-incident-notification#preliminary-examination\">preliminary examination<\/a> into the circumstances surrounding this accident.
