The Harris Review into self-inflicted deaths in custody of 18-24 year olds began its work in April 2014. It was asked to examine whether appropriate lessons had been learned from the self-inflicted deaths in custody of 18-24 year olds after the Assessment, Care in Custody and Teamwork process (ACCT) was fully rolled out in April 2007, and if not, what lessons should be learned and what action taken to prevent further deaths.

The Harris Review was published by the government on 1 July 2015. The review made 108 recommendations to the government.
